Immunization!

(여기다 올려도 되는지...)

제가 가는 학교는 Immunization 을 MMR만 요구하는데(뇌수막염은 자가진단 항목만 있고 결핵은 미국내에서 해야하는것 같더군요)
이거 애기때랑 초딩때 맞고 그랬는데
병원에 가서 해달라고 하니
작성할 수 있는 form 을 가져오라고 하더라구요
그런데 제가 가는 학교는 딱히 form 이 없고
자가진단 form 만 있고 여기에 증명서를 첨부해서 제출하라고 되어있어요
For measles, otherwise known as rubeola -- 2 doses live measles vaccine administered after 12 months of age and at least 30 days apart or results of a blood test which shows you are immune to measles (copy of lab report must be provided).

For mumps, 1 live dose mumps vaccine administered after 12 months of age or results of a blood test which shows you are immune to mumps (copy of lab report must be provided).

For rubella, also known as German measles -- 1 live dose rubella vaccine administered after 12 months of age or results of a blood test which shows you are immune to rubella (copy of lab report must be provided).
그리구요,

A copy of your immunizations on physician’s letterhead, which includes printed name, address and telephone number
이런 조건이 있는데
그 병원이 작은 병원이라 그런게 없는거같구
그냥 주소와 병원이름 그리고 의사선생님 사인 이렇게 하면 안될까요

아 그리고 가장 궁금한것,
공통적인 immunization form 같은 것이 있나요??
구글에서 막 찾아봐도 없어서 ㅜㅜ
